UD, after her check up at the D!ck vet on 9th March she was back in for a CT last week (a bit annoyed at 2 seperate apps for this due to ££ but anyway)
Vet called today with the results and they are really happy with her progress, the infection in her nose is all but gone, her ears are significantly improved with one being almost clear and the other being 50% better than previously. So they want to admit her to have her ears flushed under GA again (its only really one that needs it but as the other one isn't 100% they'll do both)
He doesn't think that the rattley breathing is anything to do with the infection, and is irrelevant to that, they've found she has one very narrow windpipe and thinks thats what is causing the rattling but isn't concerned about it.

CT Investigation: Study: Whole body (mousetrap) - compared to 22/12/2021
Given History: Re-checking for progression of rhinitis, dental disease and otitis.
Previous information:
CT scan performed December 2021. Bilateral myringotomy performed 30th December 2021.
Imaging Findings:
Head:
The previously described right sinonasal changes have markedly improved, with only remnant presence of non enhancing material within the ventral recess of the right maxillary sinus. Mild persistent lysis of the ipsilateral rostral nasal turbinates.
Both tympanic bullae (Rt>Le) are only partially obliterated by fluid, and their osseous walls remain smoothly thickened. Improved.
No significant dental pathology is observed.
Thorax:
The caudal cervical/thoracic inlet trachea is mildly to moderately circumferentially narrowed compared to its more cranial and thoracic portions, and also compared to previous study. There is no clear evidence of tissue proliferation to explain this finding, but assessment may be limited by the small size of the patient and the relative subtleness of these changes. At the narrowest point, the tracheal lumen measures 2.9x2.7mm, whereas more cranially it measures 5.1x4mm.
The remaining structures of the thorax are unremarkable.
Abdomen:
Minimal amount of mineralised urinary sediment within the bladder.
Diagnosis:
Caudal cervical tracheal narrowing. Ddx inflammatory stricture, artifactual due to mucous accumulation, less likely neoplastic proliferation. New finding.
Improved right mildly destructive rhino sinusitis.
Improved bilateral otitis media (grade I).
I would consider an anaesthetic to repeat the myringotomy / middle ear flush on the right side and also repeat on the left as we are there.
I suspect the noisy breathing is due to a functional alteration in air flow due to the change in tracheal diameter.

Belle had her second ear flush today, I was expecting a call after the procedure but never got one (I knew she'd be staying in for 24-48hrs tho). However her discharge notes have been emailed through so I assume it went fine!
Luckily she doesn't need to get anything in her ears this time, but she's on Loxicom & Emeprid for 5 days and baytril for 4 weeks.

She seems to be fine.... although she is still a bit snotty but apparently her ear infection is as good as gone and we've just to keep nebulising

Belle went in for a follow up CT scan in December and her ear infection and respiratory infection are back
This time they gave us 3 options for treatment; another ear flush, surgery or palliative care.
I'm going with another ear flush as they said its still a reasonable option, and surgery seems excessive if not necessary

She's booked on for Tuesday (10th), mean time she's finished her course of antibiotics and isn't taking her loxicom too badly, she still needs to be caught for it but she's not fighting too much.

Both of them don't even like being touched never mind snuggles

Her procedure went fine, she was kept in overnight for observation and home the next day looking bedraggled but munching away happily. They managed to get a lot of stuff out the right ear but not so much the left. She's behaving for taking her oral drugs but not the ear ones - 1ml antibiotics per ear, 1ml is a lot for a little ear :|
Got to go back for a check up in a week or so.

Ear drops are miserable for humans so buns must hate 'em. Good luck administering them.
