Should i be worried about this?
It's a lump, it's hard, and it's always been there, he has one in the same place on the other side, but it's never been visible or red like that :/
He's fine in himself, he's had some smelly poos lately but that's not unusual for him, his routine has changed a bit recently, and I've been out more so i put that down to a bit of stress

I can't really see what we are looking at.
I am assuming that it isn't the top of his scent gland vents?
If not I think is is the old addage that it is best checked out by a vet.

It's not his now empty sack of goodies is it? Max had something similar after having his removed but some jollop from the vet sorted it out. Sparky will know the terminology....

Max had one swollen (ex)testicle for a few days after his neuter (a hematoma - basically a bit of bleeding into the tissue under the skin). It's hard to see on Hector without actually handling him. Is it right under the tail, Zo, where his testicles would have been? When you say it has always been there, do you mean you noticed it before he was neutered too?

Its difficult to see but I agree with Cheryl.
Is there any possibility that his scent glands/vents need a clean? Personally id check those and then get him checked by a vet next week. x
